Take a clear photo or scan. A parent or guardian can upload it using the form below.

This form must be completed by a parent or guardian aged 18 or over. We only ask for the details needed to administer the competition and contact winners.

Who can enter: the competition is open nationwide to children living in the United Kingdom. A parent or guardian aged 18 or over must submit the entry.
Entry limit: one entry is permitted per child. Duplicate entries for the same child will not be accepted.
How winners are chosen: three valid entries are selected at random. The first selected entry wins the £50 voucher and the next two selected entries each win a £25 voucher.
Who we contact: Astley Digital will only contact the parent or guardian whose details appear on the winning entry.
Children’s privacy: please provide the child’s first name only. Do not include an address, school name or other personal information in the form or photo.
